Cannabidiol (CBD) is just one of more than 100 chemicals called phytocannabinoids that are present in the Cannabis Sativa plant. CBD is considered to be the second most major component, behind the psychoactive chemical, THC. Over the past several years, CBD has become the most sought-after natural medicine on the market. Despite its popularity and its proven effects to treat chronic pain, anxiety, and depression, scientific research of the effect that CBD may have on substance abuse and addiction is lacking. 

Substance Abuse and Mental Health 

When one thinks about mental illness, your thoughts may automatically focus on anxiety disorder, depression, or phobias, but what you may not know is that substance abuse is a serious form of mental illness. Habitual abuse of drugs and alcohol challenges and changes typical desires and priorities. It changes normal behavior and interferes with one’s ability to go to work and/or school, and to have a good relationship with friends and family. Substance abuse disorders are socially and financially burdensome on families and is considered a chronic illness if gone untreated. 

In 2014, the National Institute of Mental Health (NIMH) stated that 20.2 million adults in the United States had a substance abuse disorder and that, of those, 7.9 million had both a substance abuse disorder and another mental illness. Over half (4.1 million) of the people with both a substance abuse disorder combined with another mental illness were men. Having two illnesses at the same time is known as “comorbidity” and it can make treating each disorder more difficult. 

CBD, Substance Abuse, and Science 

In contrast to the aforementioned proven effects that CBD has on conditions such as chronic pain, anxiety, and depression, the exploration and scientific research of the effects of CBD on substance abuse behavior is extremely limited. In an article from 2017, published by the National Center for Biotechnical Information (NCBI) by PubMed, it is stated the CBD has the potential to enhance the effectiveness of treat of a wide range of neurological and psychiatric disorders. The article focuses on studies that have used experimental procedure with clinical relevance for understanding the mental and physical mechanisms involved in the treatment of anxiety-related and substance abuse disorders. 

Pursuant to the article, CBD works along with the endocannabinoid system, when given to test subjects acutely, resulting in reduced fear of memory expression explaining its effect on anxiety and depression, while revealing CBD as a potential candidate as a viable treatment for phobias and PTSD. However, contrary to the studies of the effects of CBD on anxiety and learned fear, the results of the same treatment on addictive drug memory processing have barely gotten off the ground, though anecdotal evidence is significant and growing. 

Considering the aforementioned comorbidity between anxiety and substance abuse disorders, CBD, according to this review, should also be explored as a treatment for substance abuse. One pressing issue is determining the effects of continuous CBD treatment on different emotional memory processes. This review strongly concludes that a potential therapeutic strategy is to use CBD on a consistent basis to reduce symptoms by putting a damper on fear and/or drug memory expression. The review goes on to say that, “CBD given acutely during the psychological therapy session aimed at impairing memory reconsolidation or enhancing extinction might be sufficient to facilitate this effect”. 

More research is necessary to explore the benefits of CBD in all classes of abused drugs in all three stages of addiction; drug classes being stimulants (cocaine and amphetamines), opiates (heroin and morphine), and other drugs (alcohol and tobacco), and the three stages of addiction being intoxication (when the drug produces positive rewarding experiences), withdrawal (when the user experiences acute physical and mental withdrawals symptoms), and relapse (when the user experiences cravings and is at risk of drug-seeking behavior after a period of abstinence).
